This Apollo 13 insurance cover is signed by James Lovell, Ken Mattingly, and Fred Haise. Ken Mattingly was replaced by John Swigert who was grounded by the flight surgeon after exposure to German measles. 

The craft was launched on April 11, 1970 from the Kennedy Space Center, Florida. Apollo 13 was the seventh manned mission in the American Apollo space program and the third intended to land on the moon but never made it there due to severe technical problems that came close to costing the crew their lives. Luckily the crew made it home to earth safe and the sale of this insurance cover was never needed to support their families.

These famous insurance covers produced immediately prior to space missions in order to provide financial security for astronauts' families in case of disaster, have earned near-legendary status as the ultimate space-related philatelic item, and exist as one of the most sought-after formats for astronaut autographs.
